What You'll Do:
Manage a patient caseload, proactively addressing both clinical and non-clinical care gaps.
Identify and coordinate community resources that can enhance patient care and support.
Educate patients on chronic condition management, helping them improve their health literacy.
Assist with medication management, including reconciliation, adherence, and refill coordination.
Ensure timely delivery of essential services, such as Home Health, DME, and Home Infusion.
Communicate effectively with patients to resolve questions and understand their needs.
Manage referrals and schedule appointments to ensure seamless care coordination.
What You'll Need:
Active Multi-State/Compact License (LPN/LVN) (NLC)
Strong technical skills (Microsoft Office Suite)
Excellent written and verbal communication skills
A dedicated home office or HIPAA-compliant workspace
High-speed internet for seamless remote work
Preferred Qualifications:
Experience with Epic electronic health records
Bilingual candidates are highly encouraged to apply
Additional single-state LPN licensures
