Job Overview:
Our client is seeking a compassionate Home Health Medical Social Worker to join their care team. This position requires a Master’s in Social Work and relevant experience in patient care planning and support. The role involves collaborating with healthcare professionals to deliver care and support patients in the Albion, MI area, with occasional travel to surrounding locations like Jackson, MI .
Key Responsibilities:
Collaborate with the healthcare team to manage and deliver patient care plans
Provide emotional and social support to patients in the Albion, MI area and surrounding locations
Maintain accurate and timely documentation of patient care in accordance with organizational standards
Work independently while managing time and schedule effectively
Travel up to 20% of the time to visit patient locations and provide care in their homes
Utilize tablets, mobile phones, and Electronic Medical Records (EMR) software to track and update patient progress
Ensure timely compliance with deadlines for charting and patient documentation
Qualifications:
Required:
Master’s degree or higher in Social Work
1+ years of experience in Social Work, preferably in a healthcare setting
1+ years of experience with Electronic Medical Records (EMR), including TruChart EHR or other systems
Valid MSW (Medical Social Worker) license in the state of Michigan (MI)
Valid Driver’s License, reliable mode of transportation, and auto insurance
Willingness to travel as required for the position (up to 20%)
